Monitoring form for LBoC Legacy Career Development grants

Thank you for applying to the LBoC Legacy Career Development grant scheme.

This monitoring form is so that we can check that the grant is reaching a spread of communities across Croydon. This monitoring form is anonymous, and we have no way of connecting it with your application, so if you want to be considered for the Deaf, disabled or neurodiverse category of the scheme, make sure you tell us on your application by answering question four.

Disability

The Equality Act 2010 defines someone as a disabled person if they have a physical or mental impairment which has a long term and substantial adverse effect on their ability to carry out normal day to day activities.        

A disability may include progressive conditions such as HIV and cancer, mobility, sight or hearing impairments or mental health issues such as depression.
