In Croydon we recognise that the journey from adolescence to adulthood is a time of significant change and challenge for all involved and in particular for the young person themselves. Having a special educational need and/or disability can add to the complexity of this. In developing this Preparation for Adulthood Policy with our community we aim to effectively support all young people with special educational needs and/or disabilities (SEND) as they mature into the adults they want to be.​

As a Local Area we are ambitious to deliver high quality support and services to enable smooth transitions throughout each stage. We support and promote person-centered transition planning with the person at the heart of it. This must begin at the earliest opportunity so that all key partners can provide the relevant support as and when required.  Our Preparation for Adulthood Transitions Multi-Disciplinary Protocol outlines our statutory duties, joint working and commissioning practices to support children and young people on their journey to adulthood. This includes information on pathways and support to aid transitions at the earliest stage.​
