Your Independent Reviewing Officer (IRO) is a social work professional who looks at the plans being made for you to be sure the right things are happening and you are being well looked after. It is very important that your IRO and social worker know how you feel about your life and where you are living at the moment. Your ideas and thoughts will help in planning your review meeting. If you prefer to draw a picture or share a photo, then there is space for you to upload this at the end.
This form will be saved to your file and can be seen by people who attend your review meeting. Please speak to your social worker or IRO if there is anyone you don’t want it shared with. Sometimes IRO managers might use information from the forms to learn about how we are doing in general, in providing a good service to looked-after children. This would always be done anonymously, so your data would still be confidential.

Personal data submitted (including names, addresses, contact details and any equalities data) will be processed in accordance with the requirements of the UK General Data Protection Regulations. Data will only be used for the purposes of the consultation. Details will not be shared or published. By participating, you agree to your data being used in this way.
