We want to find out what you think about Covid-19 vaccinations and if you have seen any of our videos and information via social media channels.

We really value your input and everyone filling in this survey will be entered into a prize draw. The winner will be able to choose from one of the following prizes:

• Parkour course for one person
• Speedboat voyage for you and a friend
• Climbing the O2 for you and a friend
• 1-hour DJ taster course
• Electric go-karting experience for you and a friend
• Escape room experience for you and a friend
